How does it work?

Our software risk stratifies all patients at practice, group and borough level, using multiple risk-stratification metrics that are updated every day. Clinicians then can use these metrics in combination with their experience to determine which patients will benefit most from support programmes and early interventions. We integrate risk stratification into every aspect of health care delivery.

For example, a practice faced with a large number of patients requiring a health check, or smoking cessation advice, can use any Health Analytics risk metric to ensure that the highest risk patients are given priority. Our systems are quick and easy to use, which means you can spend your time on what matters most – your patients.

Information governance

We work to strict NHS Information Governance guidelines and can offer anonymous risk stratification and triage of patients if required. All access to the system and activity associated with each patient is logged for audit.
